We only rarely have seen injured penguins. With very few exceptions, all the red we see is krill coloured penguin poo getting all over penguins, rocks, eggs, etc. Once in a great while we do see photos showing injuries (@yshish has a collection of these) and with experience in seeing the usually poo-marks we can see the difference with real blood.

Unfortunately, if we do observe a truly injured penguin there is absolutely nothing to be done. The photos are usually a year or more old by the time we classify them on Penguin Watch, and that penguin either recovered or didn't. Life is hard in Antarctica, but penguins are ideally suited for conditions and dangers there.
